Referrals
SEMH OutReach Referrals
Referrals are to be made directly by the school via Headteacher/SENDCo** to the OutReach Team email with a signed parental consent. These are reviewed weekly, receipt required prior to 12pm each week; Tuesday for Primary and Wednesday for Secondary. They are allocated on a case by case basis with consideration to the Inclusion pathway and SEND process of Rotherham. The aim is to then contact schools before the end of week for further information or arrange discussion.
Following this there will be an initial record of observation shared, interventions plan and a service level agreement put in place that runs for upto 12 weeks. Throughout this process we strive to stay connected to the School’s key staff for the student with evidenced graduated response and SEMH assessment.
